Register

We are happy to accept new patients who live in the postal code areas of IP3, IP4, IP5 and IP10.

To Register with Ravenswood Medical Practice you need to collect a New Patient Registration form and questionnaire from reception or download the Registration form and questionnaire from this page. Once you have completed both forms, return them to the receptionist who will then process your registration.

You need to fill in both forms for each person who you wish's to register as a patient including children.

Patients undergoing treatment or on medication will need to book an appointment with the Doctor before the next repeat prescription is due.
