If you have a 'Long Term Condition' such as diabetes, your review will usually be undertaken by the Practice Nurse.

We will contact you to arrange review of your Long Term Condition. However, if you have a query please telephone the Nurses' Secretary direct on 01986 891718.

Diabetic Reviews:

If you have diabetes there will usually be two check-ups a year with the Practice Nurse - one major check-up and a mini check-up. The week before each check-up you will need a blood test and sometimes this will need to be 'fasting' (nothing to eat or drink, except for water, for 12 hours before your test).

The nurse may check the following:

  • weight
  • blood pressure
  • vision
  • feet
  • urine (please bring a specimen passed on the same day as your appointment)

Once a year you should have your eyes photographed - either on the mobile retinal eye screening van which visits the surgery, or at a hospital clinic. If you have not had this done, please tell the nurse at your check-up. (NB: This eye test is not done at the Opticians).
