Prescriptions - save money

From 1st April 2011 National Health prescription charges increased to £7.40 per item.  If you have many medications it is more economical to purchase a pre-payment certificate (PPC - Form FP95).  First check to see whether you are entitled to free prescriptions by looking at the HC11 leaflet "Help with Health Costs"  more>>

If you have booked or are planning a trip abroad it is essential you have the most up-to-date travel health information. We are able to offer the current advice applicable to your chosen destination and where appropriate vaccination and private prescription for malarial prophylaxis more >>

Extended hours

The practice has responded to the government's drive to improve GP access for commuters and other patients who are unable to attend for a routine appointment during normal surgery hours. Access to these services is by appointment only more>>

Download new patient registration forms

We welcome registration from new patients who live within our practice boundary. To make life simpler, you can download a New Patient Registration Form and our New Patient Questionnaire from this website. You will need to download and print a copy of each form for every family member who wishes to register and then bring the forms to the surgery to complete the registration process more>>
