Patient Reference Group

The practice is setting up a Patient Reference Group.  Members of the group will be contacted by e-mail from time to time and asked questions on how we can improve the services the practice provides and how they perceive our surgery and staff.

If you are happy for us to contact you occasionally by e-mail please click on the icon below to sign-up on-line, or if you prefer you can download a copy of the form to hand in to reception at either site.  Alternatively you can ask for a copy of the form from reception. Once you have registered we will be in touch shortly thereafter!

The Claypath Medical Practice has served the city centre population since 1900. The University Health Service began in 1965, originally to cover the student body but later expanded to include staff and their families. In 1999 Claypath Medical Centre and the University Health Centre merged to become one practice.

The list size currently stands at circa 24,500 patients, approximately half of which are University students.
